These Terms & Conditions govern your use of ltewellnessclub.com and communications from LTE Wellness Club. LTE Wellness Club is operated by LTE Wellness Club LLC (“LTE Wellness Club,” “we,” “us,” or “our”), 12425 Taft Street, Pembroke Pines, FL 33028. By using this website or opting in to our messages, you agree to these terms.

1. SMS/Text messaging terms

Program description. LTE Wellness Club sends two separate types of text messages, each requiring its own consent:

• Non-marketing (informational) messages — appointment confirmations and reminders, schedule-related communications, and replies to your inquiries.
• Marketing messages — special offers, discounts and service updates.

Opt-in. You enroll by checking the corresponding consent checkbox on one of our website forms (or by otherwise giving us your express written consent) and providing your phone number. Each checkbox is optional and independent: you can consent to one type of message, both, or neither. Consent is voluntary and is not a condition of purchasing any goods or services, booking a session, or submitting a form.

Message frequency. Message frequency may vary depending on your interactions with us.

Message and data rates. Message and data rates may apply. Charges are billed by your mobile carrier according to your plan.

Opt-out. You can cancel at any time by replying STOP to any message from us. After you send STOP, we will send a final message confirming that you have been unsubscribed, and you will no longer receive messages of that type from us. If you want to join again, you can opt back in at any time by signing up through our forms or by texting us your request.

Carriers. Carriers are not liable for delayed or undelivered messages.
